Admissions Care Coordinator (RN)
Title:Admissions Care Coordinator (RN)
Shift:4 10-hour shifts

$3,000 sign on bonus available!

4-10 hour shifts M-TH or T-F (8:00 AM-6:30 PM)

Coordinates and completes the referral and admissions process.  Assists with and performs projects and activities for marketing services as needed
Develop and maintain good working relationships with physicians, hospital and facility staff and other referring agencies
Assess the physical and emotional needs of the patient/family to provide the framework from which the initial care plan is developed
Work closely with the billing department to verify insurance information is accurate 


Computer charting/documentation experience
Excellent written and verbal communication skills
Valid Driver’s license required
Valid Florida RN license required
Hospice and/or Admissions experience a plus!
CHPN preferred
